Compassionate Fee – $25.00

In order to quality for the compassionate fee for the initial visit, patient must show proof of one of the following terms. Fee will be paid once verified in our office.

• Full Disability

• Veteran

• Medicaid

To qualify for a Medical Marijuana Use Registry identification card, a patient must:

  1. Be a permanent resident of Florida with proper Identification
  2. Be entered into the Office of Medical Marijuana Use Registry by a qualified physician
  3. Be diagnosed with a qualifying medical condition,
  4. Submit a completed application to the Office of Medical Marijuana Use Registry

Qualifying conditions include:

  1. Acquired immune deficiency syndrome (AIDS)
  2. Amyotrophic Lateral sclerosis (ALS)
  3. Cancer
  4. Crohn’s disease
  5. Epilepsy
  6. Glaucoma
  7. Parkinson syndrome
  8. Post-traumatic stress disorder (PTSD)
  9. Multiple sclerosis (MS)
  10. Intractable seizures
  11. As well as any other debilitating medical condition of the same kind or class as or comparable to those enumerated, and for which physician believes that the medical use of cannabis would likely outweigh the potential health risks to the patient.

Medical marijuana is available in Florida, however, remains illegal under federal law.
